This Privacy Policy also explains the use of cookies and similar technologies on the Site. Your access to and use of the Site constitutes acceptance of this Privacy Policy and consent to our collection and use of your information as outlined below.
As described in additional detail below, we are not a medical group. Any telemedicine consults obtained through our Site are provided by independent medical practitioners including licensed healthcare providers (each, a "Provider"). Your Provider is responsible for providing you with a Notice of Privacy Practices describing its collection and use of your health information, not us. If you do not agree to be bound by those terms, you are not authorized to access or use our Site.
Please note that the Site is directed towards users who reside in the United States. It is not our intent to gather personal information from individuals residing outside the United States.
Our Site is not intended for or designed to attract children under the age of 13. We do not knowingly collect any personal information from anyone under the age of 18 without the consent of a parent or guardian, and you must be 18 years of age or older to submit registration or survey information. If we learn we have collected or received personal information from a child under 18 without verification of parental consent, we will delete that information. In addition, when you request treatment from a Provider through the Site, you may share personal information, medical information submitted for diagnosis or treatment purposes (including information about your applicable medical history) and current complaint or other reason for visiting a Provider. The information you provide to a Provider through the Site may be protected under applicable federal and state laws applicable to health information, including but not limited to the Health Insurance Portability and Accountability Act of 1996 (collectively, "HIPAA").
We will not use or disclose information about you that is protected under health information privacy laws except on behalf of the Providers or as permitted by such laws.
We may collect information directly from you when you visit, access or use the Site or when you register an account with us. We may also collect information when you purchase a Service or any Products available on our Site, submit information through a survey, communicate with us or post or submit content on the Site.
Like most website operators, we use cookies and similar technologies, local shared objects, Software Development Kits, and pixel tags that automatically receive and track certain data about how you and other visitors interact with our Site.
"Cookies" are small text files that are stored on your browser or device by websites, mobile apps, online media and advertisements. There are different types of cookies including first-party cookies, third-party cookies, session cookies, and persistent cookies.

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
You understand that Ozari Health is not a "covered entity" as defined by HIPAA. The Pharmacies and Providers may be considered a "covered entity" or "business associate" under HIPAA, and Ozari Health may, in some cases, be a "business associate" of a Pharmacy or Provider. If Ozari Health is deemed a "business associate," we will be subject to additional protections of your "protected health information," as defined under HIPAA ("PHI"). Your PHI will be used and disclosed only in accordance with applicable laws, regulations, and contractual protections.
We respect your right to make choices about the ways we collect, use, and disclose the information you provide on or through our Site. If you don't want information about your visit to our Site sent to Google Analytics, you may download an Opt-out Browser Add-on. You may also opt out of interest-based advertising through the Digital Advertising Alliance's tools.
You may change previously expressed preferences regarding how we use information you provide. For example, if you receive periodic emails from us, you may decide to stop receiving those emails by clicking the "unsubscribe" link in an email's footer. You may also change preferences through your account settings.

We will respond to you within a reasonable time and, in any case, within the time limits established by applicable law.
